Rehabilitation plan

Rehabilitation means developing your skills and capabilities and creating a living environment that supports independent coping.For rehabilitation, a rehabilitation plan will be drawn up for you that gives an assessment of your operational capacity and your need for help and lays down rehabilitation activities.

The rehabilitation plan

gives you a thorough summary of your rehabilitation possibilities helps the medical assessment committee or the medical expert correctly determine the degree of severity of your disability and assess the need for additional expenses

Preparing the plan

the rehabilitation plan is prepared by a team at the rehabilitation institution with your active participation (preparing the rehabilitation plan may take up to two weeks)
if you are applying for the degree of severity of your disability to be determined, the medical assessment committee or a medical expert may refer you to the rehabilitation institution to have your rehabilitation plan prepared
you may also turn to the rehabilitation institution by yourself

Contents
the rehabilitation plan includes an evaluation of your operational capacity and a plan of action
it contains the activities necessary for your rehabilitation, their frequency, duration and the people providing them in carrying out the activities contained in your rehabilitation plan, you will be guided and supported by a social worker, your family physician, case worker or another person or institution that co-ordinates the carrying out of the rehabilitation plan the specialists in the rehabilitation team will help you find and get in touch with the person who co-ordinates carrying out the rehabilitation plan

Carrying out the plan
if you applied for the degree of severity of your disability to be determined, the rehabilitation plan will be sent to your home together with the decision determining the degree of severity of your disability if you turned to the rehabilitation institution by yourself, make arrangements with the institution for receiving your rehabilitation planonce you have received your rehabilitation plan, you can start carrying out the activities prescribed in the plan when you have carried out an activity, a respective note will be made in the plan of action

Assessing the outcomes
when the next rehabilitation plan is prepared for you and the degree of severity of your disability determined again, the fulfilment of the plan of action and the outcomes of rehabilitation will be assessed

Payment
if a medical assessment committee or a medical expert referred you to the rehabilitation institution to have your rehabilitation plan prepared, the state will pay for the preparation of the plan in other cases, you will have to pay for the rehabilitation plan yourself when paying for the activities prescribed in the rehabilitation plan, you can use the social benefits for disabled persons (disabled adult allowance, disabled child allowance, rehabilitation allowance, in-service training allowance) county governments will pay for children's rehabilitation plans in accordance with their financial possibilities employment offices and the health insurance fund will pay for employment and health services, social services will be funded by the state and local governments in accordance with their financial possibilities

For your information
the rehabilitation plan must be prepared by a team which includes:

• a social worker

• a medical specialist

• a nurse

• a psychologist (psychologist/vocational counsellor)

• a speech therapist/special education teacher(teacher)

• a physiotherapist

• if necessary, other specialists will be included

the rehabilitation institution is obligated to explain to you how the plan will be prepared you have the right to know who, and for what purpose, will use the data you provided carrying out the activities of the rehabilitation plan that are covered financially is your responsibility
